  • If completing the form by hand and forwarding by email – Please download the form using the link button below . Print and fill in the form. It must be signed by both the applicant and sponsor. Scan and save the form as a .pdf or .jpeg file to your computer, and email the attachment to secretary@shropshirewelfaretrust.org
  • If completing the form by hand and forwarding by post – If it is not possible to send an application by electronic mail, please email or telephone the secretary for details of a postal address that you can use to forward the completed hard copy.

PLEASE NOTE THAT FOR ITEMS PURCHASED, OR WORK UNDERTAKEN, FOLLOWING THE PROVISION OF A GRANT, THE CHARITY MUST RECEIVE A COPY OF THE RECEIPT/S OBTAINED, WITHIN 2 WEEKS

COVID 19 ARRANGEMENTS – the charities recognise that due to social distancing, it is not always possible for both the sponsor and applicant to sign the application form. Where a form is completed by the applicant, a supporting email from the sponsor will be acceptable. Where a form is completed by the sponsor, an email from the applicant saying that they accept the conditions of the charity, as laid out on the application form, will be acceptable.
